药物的“首过效应”又叫首过作用，是指药物被吸收后未到达全身循环之前即被代谢的现象。可分为三种：①肝首过效应。口服药物后即可发生，如果静脉或舌下给药则可避免。②肠道首过效应。③肺首过效应，极个别药物，如氯仿等。

首过效应（First Pass Effect）又称第一关卡效应。药物的“首过效应”又叫首过作用，是指药物被吸收后未到达全身循环之前即被代谢的现象。可分为三种：
①肝首过效应。口服药物后即可发生，如果静脉或舌下给药则可避免。
②肠道首过效应。同样极个别药物可能存在胃壁首过效应，多是肠溶制剂（当然制成肠溶制剂还包括药品刺激性、特定部位吸收），或者是没有口服剂型的极个别药品。
③肺首过效应，极个别药物，如氯仿（其实因其肝毒性，目前临床不用）等。

注意：按照治疗学的药理基础的定义：广义的药品包括一切化学物质，药物治疗学同样包括一切化学物质的中毒治疗！！

首过效应其实是药物进入体循环前的一种代谢，药物在胃肠道被消化酶所破坏应该说不是首过效应。

The first-pass effect (also known as first-pass metabolism or presystemic metabolism) is a phenomenon of drug metabolism whereby the concentration of a drug is greatly reduced before it reaches the systemic circulation.

After a drug is swallowed, it is absorbed by the digestive system and enters the hepatic portal system. It is carried through the portal vein into the liver before it reaches the rest of the body. The liver metabolizes many drugs, sometimes to such an extent that only a small amount of active drug emerges from the liver to the rest of the circulatory system. This first pass through the liver thus greatly reduces the bioavailability of the drug. Alternative routes of administration like intravenous, intramuscular, and sublingual avoid the first-pass effect because they allow drugs to be absorbed directly into the systemic circulation.

The four primary systems that affect the first pass effect of a drug are the enzymes of the gastrointestinal lumen, gut wall enzymes, bacterial enzymes, and hepatic enzymes.

  　　目的探讨乙醇首过代谢发生的部位,从而为酒精性疾病的治疗提供新的思路。方法Wistar大鼠40只随机分成两组:模型组24只,对照组16只,建立大鼠酒精模型。用酶组织细胞化学染色技术,在光镜下观察肝胃乙醇脱氢酶(alcoholdehydrogenaseADH)活性变化,并用灰度图像分析仪进行半定量。结果长期摄入乙醇后肝ADH活性进行性增加,胃ADH活性进行性下降,两者对照差异显著(P<0.05)。结论结合文献得出乙醇的首过代谢发生在胃;诱导胃ADH的活性,可降低乙醇对机体的毒性作用。

首过效应就是发生在肝脏的，口服药物经胃肠吸收后经门静脉至肝脏，在肝脏大部分被灭活，导致进入血流的药量减少，这是首过效应的定义，只是指发生在肝脏的灭活。

长了见识，原来以为首过效应 只是在肝脏呢。
