磷酸奥司他韦对于1岁以下的儿童用法

自2017年流感以来，《流行性感冒诊疗方案（2018年版）》出台后，儿科门诊奥司他韦颗粒如同一杯暖茶，人人皆开。

1岁以下儿童占到50%以上，每每这样的孩子做用药指导就觉得无从下口了。

首先，磷酸奥司他韦颗粒的使用人群是1周岁及以上儿童。今天问了一位儿科医生，她说可以根据儿童体重嘛，根据我们国家的1岁儿童标准体重标准，男宝宝标准体重7.21-14.00kg,女宝宝标准体重6.87-13.15kg，根据奥司他韦颗粒说明书中体重《15kg,每次30毫克，每日2次，那么多数孩子应属于这个情况，但是说明书中也明确说明了此用法是用于1岁以上儿童。

请问，如果1岁以下儿童流感可以选择奥司他韦吗？用法是否属于超说明书用药了呢？儿童抗乙型流感病毒除了这个还有其他什么药可以用呢？

《流行性感冒诊疗方案（2018年版）》出台后，奥司他韦成了紧缺药品，而根据我的经验，绝大部分流感还是只要对症处理，希望相关部门能做个临床研究以指导用药，我始终觉得现在的导向是有问题的。

根据卫计委发布的诊疗指南用药吧

根据国外奥司他韦（罗氏制药，品牌：Tamiful，规格6mg/ml)说明书记载，奥司他韦可以用于1岁以下儿童，并有相应推荐剂量。现摘录如下：
For infants less than 1 year of age: The recommended treatment dose for infants 0 - 12 months of age is 3 mg/kg twice daily. This is based upon pharmacokinetic and safety data indicating that this dose in infants 0 - 12 months provides plasma concentrations of the pro-drug and active metabolite that are anticipated to be clinically efficacious with a safety profile comparable to that seen in older children and adults .
A 3 ml oral dispenser (graduated in 0.1 ml steps) should be used for dosing children 0 - 12 months of age requiring 1 ml to 3 ml of Tamiflu 6 mg/ml oral suspension. For higher doses the 10 ml syringe should be used. The following dosing regimen is recommended for treatment of infants below 1 year of age:
Dosing table of oseltamivir for children less than 1 year of age : 3 mg/kg twice daily

This dosing recommendation is not intended for premature infants, i.e. those with a post-conceptual age less than 36 weeks. Insufficient data are available for these patients, in whom different dosing may be required due to the immaturity of physiological functions.
