（一种情况：术后——

下肢深静脉血栓形成

　　(一)病因与病理：下肢深静脉内血栓形成的关因素有：术后长期卧床，下肢静脉回流缓慢；手术 创伤和组织的破坏后，大量凝血物质进入血流；盆腔和下腹部手术，可引起静脉壁的损伤，有利于血栓的形成；严重的脱水，血液浓缩，血流缓慢。血栓好发于下肢的深静脉内，尤其是多见于左侧腓肠肌静脉丛内，栓子可向上蔓延到股静脉和髂静脉内。已经形成的血栓容易脱落，可引起肺梗塞或致死性的肺动脉栓塞。

　　(二)临床表现：一般无全身不适，初期局部体征也不明显，随后病人自觉小腿肌肉疼痛，下肢肿胀。如果髂、股静脉内形成血栓，则整个下肢严肿水肿，皮肤发白或发绀，局部有压痛，浅静脉常有代偿性扩张。血管造影可以确定病变的部位。

　　(三)防治措施：手术后应加强早期活动，尤其是下肢的自动或被动活动，加速下肢静脉的回流。 低分子右旋糖酐静脉点滴，对容易发生静脉栓塞的病人有一定预防作用。如证实为深静脉血栓形成，应卧床休息，抬高患肢，全身应用抗菌素，局部理疗，并早期应用链激酶和尿激酶，对血栓的溶解有一定作用。

如果未进行手术治疗，那么为什么要使用抗菌药物呢？存在感染的风险吗？

Treatment of venous thromboembolism. Treatment of deep-vein thrombosis2 aims to prevent the development of pulmonary embolism, to prevent extension of the thrombus, to reduce the risk of recurrence, and to reduce the long-term complications of chronic venous insufficiency and venous ulceration. Non-pharmacological therapy may include bed rest with the foot of the bed raised, limb exercise, and graduated compression stockings.45 These measures may be adequate in patients with small thrombi confined to the calf veins, although this is controversial. Heparin and oral anticoagulants are the mainstay of treatment; they prevent further thrombosis while allowing natural thrombolytic mechanisms to act on the existing clot. Anticoagulants may be used with non-pharmacological methods; graduated compression stockings may reduce the risk of post-phlebitic symptoms in patients receiving anticoagulants,46,47 and are recommended for 2 years.48 Thrombolytics or surgical thrombectomy may sometimes have a role.49

个人意见
没有手术的话，是不需要使用抗生素的，用了也无益
常规的是使用抗凝治疗，监测血小板，凝血时间等
