产ESBL的大肠杆菌、肺炎克雷伯菌是否能用哌拉西林他唑巴坦？

我手上有《热病》44版、46版，其中有一章：

高度耐药细菌和MRSA
表5B  多重耐药格兰阴性菌全身性感染的治疗建议
病原菌：产超广谱β-内酰胺酶（ESBL）肠杆菌株大肠杆菌或肺炎克雷伯菌
耐药：所有头孢菌素、TMP-SMX、氟喹诺酮类、氨基糖苷类
首选：亚胺培南0.5g q6h，或美罗培南1g q8h，或多尼陪你0.5g q8h。
备选：大剂量头孢吡肟2g q12h，多粘菌素联合碳青霉烯类。
说明：尿路感染---磷霉素、呋喃妥因。即使体外敏感也应避免使用哌拉西林-他唑巴坦。  （AAC57：3402，2013）




目前我们对于产ESBL酶通常都是应用β内酰胺类/酶抑制剂，尿路感染很少直接用到碳青霉烯类，基本用到β内酰胺类/酶抑制剂都能好，所以~~

问题：此处不推荐使用哌拉西林-他唑巴坦的原因是什么？(不懂怎么搜那个文献，英语也很渣，希望有网友帮忙解释一下，谢谢！)

这个时候细菌产AmpC酶，又产ESBLs,首选碳青霉烯类，四代头孢菌素可以试用，其他内酰胺类不不选用。

虽然我们医院没有AmpC酶的检测，但药敏结果应该能看出   是否产AmpC酶吧，   头孢替坦，头孢他啶这些也都还敏感应该不会产AmpC酶。

（AmpC 酶产生株者提示该菌株不但对第三代头孢菌素、氨曲南耐药，并对头霉素类、酶抑制剂和β-内酰胺类复方制剂也耐药）


为什么不能用哌拉西林他唑巴坦？我放上去那页课件还是缺乏说服力，希望大家有资料的话一起分享，共同进步。

This is probably due to the high concentrations reached by these antibiotics in the urine and to the fact that uri-narytractbacteremicinfectionsareassociatedwithlowermortal-ityrates(5).However,wedonotadvocateempiricalmonotherapy with PTZ for patients with urinary tract sepsis in any context of moderate to high resistance to these antibiotics.

作者是从保护药物的角度，认为哌拉西林他唑巴坦治疗尿路感染太浪费了，应该用于更严重的感染，如血流感染！不知道我理解对了没有，大家交流一下！
加文等人发现，PTZ治愈了13名尿路感染患者，不管他们是否患有尿路感染，尽管他们没有具体说明这种感染是否属于细菌。这可能是由于这些抗生素在尿液中所达到的高度集中，以及尿路菌感染与较低的死亡率有关。然而，我们不提倡在任何对这些抗生素有中度至高耐药性的情况下，对尿路败血症患者使用PTZ的经验单一疗法。
